If a child needs a prescription medication administered at school, a form needs to be signed by the doctor and guardian.
If a child needs an over the counter medication administered at school, a form needs to be signed by the guardian.
All medications have to be brought to school in its original packaging or official prescription container labeled with student’s name, dosage, and administration instructions. Some pharmacies will provide a second container for school use.
Any changes in the type of drug, dosage or time of administration must be accompanied by new parent and physician permission signatures and new or newly labeled containers.
ALL medications must be kept in the nurse’s office during the school day.
Please call the front office and let them or the nurse know that you will be sending medication with your child to school that day.
